Review Timeline Process

  • There are no deadlines. Applications are accepted throughout the year and are reviewed in the order they are received.
  • Please note that it takes 3-4 months for requests to be reviewed. Requests are reviewed by a Committee before it is considered by the Board of Trustees.
BEFORE YOU BEGIN THE ONLINE APPLICATION FORM
  • Application questions can be viewed or printed in advance of submitting your request. Click on the “Printer Friendly Version” located at the top right corner of the screen, after you Log-In. You can save your application and submit it when completed.
  • You may copy/paste answers from your word documents.
  • Please note that you will receive an automatic email confirmation once your organization’s application account has been created.
  • If you choose to save the application and finish later, you will also receive an automatic email stating that your application has been saved successfully, and the email will list the tracking number.
  • Once you complete and submit the application, you will receive an email that your organization’s application has been submitted successfully, and lists the tracking number.
  • If you do not receive any of these emails, please check your spam folder and add mail@grantapplication.com to your safe senders list to ensure you receive all system communications.
  • The Foundation requires grant requests to list measurable outcomes. This is not new, as the Foundation has always asked for measurable outcomes in the grant application, however, it is now a requirement. By providing measurable outcomes, it will help us better understand the impact of the purpose of the grant. It will also give us a better idea of who the organization is serving and how it plans on providing the services.
  • Please be sure to create outcome measures that are reasonable and realistic, and based on information that will help you to best assess the impact of your grant. You will be asked to report on progress made towards your outcomes when reporting on the grant, if funded, at the 12th month of your grant year.
  • If you are requesting general operating support, please list a maximum of four measurable outcomes that are most relevant to the issue(s) your organization addresses.

Please Note- You will be asked to upload the following documents:

  • Organization Budget
  • Program Budget (Include revenue & expenses for the program. Please upload a program budget narrative if items are not self-explanatory.)
  • List of Other Funding Sources (Amount, Funding Source, Date and Status - pending, or committed/received)
  • IRS Form 990 or 990 E-Z. Please include all schedules & attachments. (NOTE: we do not accept the 990-N or 990-T.)
  • Most Recent Audited Financial Statement. Please include all pages and accompanying notes. (For organizations with contributions in excess of $300,000 a year, per the requirements of the IL Attorney General, the Foundation requires a FULL Audit. NOTE: We do not accept Audit Compilations or Reviews.)
  • Current Board of Directors List (Name & Affiliations)
